Advanced Practice Clinician (NP or PA)

Scheduling Coordinator

Provides exemplary customer service in facilitating all aspects of patient scheduling activities. Manages the scheduling phone queue and assists patients with initial and follow up appointments with providers. Responsible for insurance verification on all scheduled patient accounts.

Medical Receptionist

This person will provide initial patient/clinic contact. He or she must be able to perform the varied functions of this position and at the same time be helpful and cordial to all patients.

Qualifications:

Certified Medical Assistant

Clinical Support and Referral Specialist

Full-spectrum coordination and scheduling of patient referrals to internal and external agencies. This includes follow-up to ensure that referral appointments were kept and reports are received in order to ensure “closing the loop” for 100% of the referrals. As well as enters information, compiles data, and supports the clinical care team under the patient care home health model and in accordance with established policies and procedures. Acts as a backup to other departments as requested.

Registered Nurse (on-call)

This is a Relief (on-call) position, utilized as-needed with no guarantee of hours.
