REQUEST FOR QUALIFICATIONS (RFQ) FOR ARCHITECTURAL SERVICES
Notice is hereby given that the Yakima Valley Farm Workers Clinic has received a $12,000,000 Affordable Care Act-Capital Development Grant through the Department of Health and Human Services (HRSA). YVFWC will utilize this funding and its own funding to complete three projects:
81727-01 Addition to the Grandview Medical-Dental Clinic, 1000 Wallace Way, Grandview, WA.
· Total Project Cost $2,279,000; Federal Funding $2,100,000; Percentage of Federal Funding 92%; YVFWC Funding $179,000; Construction Budget $1,705,000
81727-02 Addition to the Lincoln Avenue Medical Clinic, 2205 W. Lincoln, Yakima, WA 98902
· Total Project Cost $1,303,905; Federal Funding $1,200,000; Percentage of Federal Funding 92%; YVFWC Funding $102,905; Construction Budget $935,800
81727-03 Data Center and Administration Offices, 603 West 4th Avenue, Toppenish, WA 98948.
· Total Project Cost: $8,750,000; Federal Funding $8,700,000; Percentage of Federal Funding 99%; YVFWC Funding $50,000; Construction budget $7,200,000
Yakima Valley Farm Workers Clinic is seeking from interested, qualified architectural firms a Statement of Qualifications (SOQ) to provide professional services related to the planning, design, and construction of the three projects described above.
